Privacy notice

Our forms ask for what we need to call you back:

• Request support: your name, how to reach you, the county and Medicaid plan of the person who needs support, the services you're interested in, and anything else you want to tell us.
• Refer a client: your name, organization, role and contact details, and the client's first name, last initial, county, plan and services needed. It also asks how to reach the client or their guardian, whether they know about the referral, and anything else you want to tell us.
• Apply: our general application asks for your contact details, the job you want, your availability, your qualifications, how you heard about us and your resume. The forms for our open roles ask for your name, contact details, home address and resume, and you can add a cover letter. They also ask you to confirm you understand our background checks, and to type your name and the date as a signature.

Please don't put diagnoses or medical details in a form. We'll talk about those by phone.

Your answers go to RasCare's Google Workspace account. At RasCare, only the people who handle intake and our IT administrator can see support requests and referrals. Only the people who handle hiring and our IT administrator can see job applications.

The application forms for our open roles are run by Tally, a form service. Tally keeps a copy of each application. Make, an automation service, copies each application and resume into RasCare's Google Workspace.

Staff get an email alert when a form on this website arrives. The alert gives only the type of form, your first name and a link to your answers.

This website doesn't use advertising or tracking cookies. It doesn't use analytics tools that follow you across sites.

To block spam, the forms on this website use Cloudflare Turnstile, which checks that a person is filling in the form.
